Community Health Worker
Education and Outreach
The Community Health Worker will identify the health needs, service gaps, and barriers to care for high risk pregnant women and children in areas targeted by the Ohio Infant Mortality Reduction Initiative (OIMRI) in high-risk neighborhoods in Youngstown in Mahoning County. This is a grant-funded position and is dependent upon the continued availability of funding and project success.
Job requires commitment to quality healthcare and excellence in customer service. High energy needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Canvassing/home visiting in high- risk neighborhoods
- Client reinforcement for health information
- Community relations
This position has no supervisory responsibilities.

Job involves frequent interruptions, occasional overtime, some evening, early morning, and weekend hours, and frequent travel throughout the PPGOH service area. Work is performed in an office environment as well as in a variety of community and school settings. The noise level in the work environment is generally moderate. Reliable transportation necessary.

EDUCATION and/or EXPERIENCE
LEVEL ONE: Position requires a High School Diploma or equivalent. Experience in community outreach and home visitations in addition to experience working with culturally and linguistically diverse populations preferred. Requires computer skills in Microsoft Word and database case management. Mahoning County resident and knowledge of Mahoning County community resources. Must attend and complete the Ohio Community Health Worker Training, out-of-state 4 day curriculum training and any additional ODH required training.
LEVEL THREE: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
LEVEL ONE: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to interpret bar graphs.
LEVEL TWO: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
Frequently requires creativity, problem-solving, decision-making, interpersonal skills, flexibility, and independent judgment. May occasionally require negotiation skills and use of discretion.
CERTIFICATES, LICENSES, AND REGISTRATIONS
Valid driver’s license and proof of insurance in accordance with agency and liability policy.
While performing the duties of this job, the employee is regularly required to stand, walk, talk, and hear. The employee frequently is required to use hands to finger, handle, or feel and reach with hands and arms. The employee is occasionally required to sit. Work requires occasional bending and stooping. Rarely requires repetitive motion of the feet. Must be able to occasionally lift, push and/or carry up to 30 lbs., operate a computer including keyboard, and operate multi-key telephone. During peak activity periods work in excess of 7.5 hours per day and/or 37.5 hours per week.
